Debrief Vault draws primarily from National Transportation Safety Board (NTSB) aviation mishap data, including:
- Official NTSB event records
- Preliminary and final investigation reports
- Docket documents released during investigations
These sources are authoritative records of what occurred, but they are not instructional by default. Debrief Vault exists to organize and contextualize them for learning.
Report Status and Confidence
Not all mishaps carry the same level of certainty.
Debrief Vault distinguishes clearly between:
- Preliminary reports -- early factual findings that may change
- Final reports -- completed investigations with established probable cause
Preliminary information is presented for awareness only. Instructional conclusions rely on final reports whenever possible.

This framing is designed to improve recognition earlier in future flights.
What Debrief Vault Does Not Claim
Debrief Vault does not:
